DCMH pharmacist recognized for diabetes education, support

Honored by Indina's State Office of Rural Health

(GREENSBURG, Ind.) -- Decatur County Memorial Hospital’s Ambulatory Care Pharmacist Matt Weber has received the Patient & Family Experience Award from the Indiana State Office of Rural Health.


The award is given to those who have outstanding achievements in rural healthcare.


Weber earned this award for his efforts in providing diabetes education and support to local patients and families. Last year, he established a program that provides individualized education and information to patients.


He meets with those who need consultation regarding their prescriptions and managing their chronic conditions, including diabetes, hypertension, and obesity. In his role, Matt collaborates with DCMH providers to create individualized care plans for each patient.


“We’ve had great success, and it is really exciting to see the improvements in people’s health and blood sugar and their engagement once they understand what diabetes is and how it impacts them,” Weber said.
